Transition Elevated App

Transition Elevated LogoThis app is designed to help Utah students, family members, and educators participate in the transition planning process. As a Utah student with a disability who is 14 years or older, or a family member of a transition age student, this app can help prepare for the Individualized Education Program (IEP) meeting where transition planning will be discussed and the Postsecondary Transition Plan will be created. Individuals residing outside of Utah may use this free app, but the information needed for transition plans in other states may be different.

Materials to Support App Use

Printable Version of App Questions
The paper application is intended to be used when technology isn't readily available or for youth to complete ahead of time and input information in one sitting.  The application does not save, but a pdf can be generated and emailed to individuals the student chooses.

Transition App Power Point IEP Meeting Template
This sample PowerPoint template can be used by transferring the student's information from their draft plan onto the slides. The student can then present the information at their transition IEP meeting.

Sample Completed Draft Plan
This is a sample of the Student draft plan that is created once a student has gone through the steps using the Transition Elevated App.
